Transfers

• Final high school transcript with graduation date and ACT/SAT test scores (only if you have earned less than 28 semester hours of college credit)
• Official college transcript(s) from each institution that you have attended
• Transfer clearance form
• Confidential mandatory health forms (student health portal)
