The following health forms must be completed in their entirety.   Pay close attention to the directions on each form and attach all laboratory testing results where indicated.

1 HEALTH SCREENING FORM
2 CONFIDENTIAL MANDATORY HEALTH FORM PHYSICAL EXAMINATION
3 CONFIDENTIAL HISTORY OF IMMUNIZATIONS
4 CDC HEALTHCARE PERSONNEL VACCINATION RECOMMENDATIONS

Note:  Take the CDC Guidelines with you to ensure that all forms are completed accordingly.

Once complete, please send all three forms back to WLU Student Health Services at one time.  Both the physical address and fax number are shown on the top right-hand side of each form. 

We need to have them back no later than 6-weeks prior to the beginning of class.
