The following health forms must be completed in their entirety.   Pay close attention to the directions on each form and attach all laboratory testing results where indicated.

Note:  Take the CDC Guidelines with you to ensure that all forms are completed accordingly.

Once complete, mail all three forms back together.  The address for WLU Student Health Services is shown at the top of each form. 

We must have them back no later than 6-weeks prior to the beginning of class.
