Room Reservation Request Form

Please fill out the following form and submit for review. After we have received your submission it will be reviewed by the facility manager. You will receive an email confirming your request along with room assignment.

This form is for Classroom space (Arnett, Main, Fine Arts, and Campbell), Union space (Ballroom, Student Leadership Center), Blatnik Gym & Dance Studio, and the Interfaith Chapel & Social Room. If you are in need of College Hall or Kelly Theater, please contact the College of Liberal & Creative Arts. ASRC facilities can be booked though Athletics. Library facilities can be reserved through Library staff.
