  1. Be typed on official letterhead, signed and dated by a qualified, licensed professional credentialed in the area related to the diagnosed condition.
  2. Include a specific diagnosis, related symptoms, and a history of the condition, including duration, stability, or progression.
  3. Provide recent objective findings, test scores, or clinical observations showing functional limitations and the current need for accommodations.
  4. List current medications and any side effects impacting function.

The following do not provide sufficient documentation for accommodations:

  • Brief notes from a physician or health professional that only request accommodations or provide a diagnosis without supporting details.
  • Notes on prescription pads or aftercare instructions.
  • Evaluation reports of learning difficulties that are not comprehensive or do not specify a learning disability.
  • IEPs, 504 Plans, outdated evaluations, or assessments normed on child scales.

These can be a helpful starting point, but additional documentation or evaluation may be requested.
